Discord Setup Guide
Send form submission notifications to Discord channels. Create a webhook and receive rich embeds.
Setup steps
Open Discord (desktop app or browser) and navigate to the server where you want to receive form notifications.
Right-click the channel you want notifications in and select Edit Channel. You need the "Manage Webhooks" permission for this channel.
Go to Integrations → Webhooks and click New Webhook.
Name it (e.g., "FormBlade") and optionally set a custom avatar. This name and avatar will appear as the "sender" when notifications are posted to the channel.
Click Copy Webhook URL. The URL starts with:
https://discord.com/api/webhooks/123456789/abcdef...
Copy this URL.
Log in to your FormBlade dashboard. Open the form you want to configure, go to Settings → Notifications, and click Discord. Paste the webhook URL and click Save.
Click Send test message in the Discord notification settings. You should see a rich embed appear in your Discord channel within a few seconds.
Tips
Messages are formatted as Discord rich embeds with a purple accent color. Each form field is displayed as an embed field with the label and submitted value.
Multiple forms
You can use the same Discord webhook URL on multiple forms. Each embed includes the form name in the title, making it easy to identify which form the submission came from. All alerts land in the same channel.
